Required Documents for Participation in Clinic:
- A Proof of Your Identity.
a. This proof of identity has to contain sufficient information to allow the consumer reporting agency (“CRA”) to match you with your file.
b. Here are the documents that are acceptable by the three CRAs:
i. Equifax
1. Equifax requires a copy of one of the following documents:
a. Your government issued ID;
b. Your driver’s license;
c. Your birth certificate;
d. Your passport;
e. Your Social Security card; OR
f. Your W2.
ii. Experian:
1. Experian requires one copy of your government issued ID, such as:
a. Your driver’s license;
b. Your state ID card; OR c. Your passport.
iii. TransUnion:
1. TransUnion requires a copy of one of the following documents:
a. Your government issued ID;
b. Your driver’s license;
c. Your birth certificate;
d. Your passport;
e. Your Social Security card; OR
f. Your current utility bill.
*All photocopies/scans should be clear and readable. - Proof of Current Address a. In addition to a Proof of ID, Experian requires a proof of your current address, using one of the following documents: 1. A recent utility bill;
2. A recent bank statement;
3. A recent insurance statement. - A Copy of Your Most Recent Credit Report.
a. You must bring a copy of your most recent credit report to participate in the clinic.
i. We will not be able to help you obtain your credit report during the clinic.
ii. Ideally, you will have a copy of your credit report from all – Equifax, Experian, and TransUnion. However, if you only have one/two, we will still be able to help. - An Approved Victim Determination Documentation (“VDD”):
a. You need a determination that you were a victim of trafficking made by:
i. A federal, state, or tribal government entity;
ii. Non-governmental organization of a human trafficking task force, including victim service providers affiliated with the organization, authorized by a Federal, State, or Tribal governmental entity to make such a determination; or
iii. A court of competent jurisdiction.
b. A signed statement by you attesting that you are a victim of trafficking if such statement or document is signed or certified by a representative of an entity described above.
c. The following types of documents will likely be accepted:
i. Certification letters issued by the Department of Health and Human Services (HHS);
ii. Child eligibility letters issued by HHS;
iii. Documentation of continued presence issued by the Department of Homeland Security (DHS);
iv. A T-Visa;
v. Continued Presence Approval;
vi. A U-Visas with a Form I-918 Supplemental B filled out indicating that the I individual experienced trafficking;
vii. Criminal human trafficking prosecution court records in which you have been identified as a victim of trafficking;
viii. Civil suit decisions involving human trafficking in which the survivor is identified as a victim of trafficking;
ix. Restitution orders that provide a victim with compensation after a criminal conviction for human trafficking;
x. Documents issued by State government agencies affirming an individual is a victim of trafficking;
xi. Law enforcement affidavit or other documented statement/attestation from a government entity; or
xii. Documented statement from a non-governmental organization, service provider, or human trafficking task force member that has been authorized by government entity.
